Patient Safety Impact Story: Documentation Can Prevent Litigation

Our Patient Safety INSIGHT Services and extensive resources and tools take a data-driven approach to identifying potential sources of risk and enhancing patient safety. We recently shared a story highlighting how our Patient Safety and Risk Management team mitigated risk in a member practice.

The team partnered with a general surgery practice to complete an analysis. An audit of the practice’s after-hours call documentation revealed that only 23 percent of the calls were recorded in the EHR. Our experts helped the practice build safe and reliable processes that standardized operating procedures while reinforcing a culture of safety and high reliability. Six months later, the after-hours call documentation rate had improved dramatically.
