Loss Run Request

This request form is for loss runs only. You can download a credentialing report, certificate of insurance, and other documents in the Agent/Broker Portal in the View Documents section of your client’s policy screen.

For any loss run request for a former insured or insured where you are not listed as the current BOR, please email memberservices@thedoctors.com and include a signed and dated consent to release form.
