Online Assessment to Benchmark Your Hospital’s Patient Safety Culture

The Hospital Safety Culture INSIGHT measures whether management practices, policies, procedures, and communication style support and promote patient safety or undermine it. Think of it as a diagnostic checkup that captures staff perceptions of the culture in individual hospital units in which they work.

Our risk management experts will compare your results with hospitals across the nation and provide specific recommendations to better focus your culture around patient safety. Repeating the survey at regular intervals enables hospitals to gauge the impacts of new policies, procedures, and initiatives on their patient safety culture.

All staff in the work unit(s) surveyed will be asked to respond to a series of statements by indicating how strongly they agree or disagree with them, or how frequently certain events occur. The Hospital Safety Culture INSIGHT takes about 15 minutes to complete and covers a range of factors that contribute to a culture of patient safety.

To achieve the most accurate picture of your patient safety culture, we recommend that you strive for full participation by staff members and encourage them to be candid with their confidential answers. As they move through the questions, staff will begin to understand how various aspects of day-to-day operations impact patient safety culture.

The Doctors Company offers an online version of the hospital survey on patient safety culture developed by the Agency for Healthcare Research and Quality (AHRQ) to heighten awareness of patient safety in your hospital and assess the status of the culture. The survey is an approved improvement activity under the Merit-Based Incentive Payment System (MIPS).

The self-administered survey is flexible to accommodate the demands placed on busy hospital units. Staff can complete the survey in sections as time permits and save partially completed surveys to work on later. Convenient navigation enables them to go back and move forward, as needed, to amend answers before submitting a completed survey.

Our risk managers will send a report to you via email about 15 days after your survey closes. It will contain a comparison of your results with other hospitals across the country and recommendations for specific initiatives and interventions that build on the participating work unit’s strengths and take advantage of opportunities to foster a culture of patient safety. This careful review process enables us to alert you to emerging risks and provide effective steps to minimize them.
